What Happens Next

If you are accepting this placement for the upcoming school year, please complete and return the Enrollment Form attached in the email you are receiving.

In addition, please provide copies of the following required documentation:

• Proof of residency in Wisconsin
  (examples include a current title or mortgage statement, current lease agreement, or current energy bill)

• Your child’s immunization record

Completed forms and documents may be:

• Emailed to liliana.tenuta@ktecschools.org, or

• Delivered in person to the KTEC High School office

If you have decided not to accept this placement, please complete the attached form indicating that you are declining placement at KTEC Schools of Innovation and return it as instructed.
